Description

The subject of the contract is the provision of professional development supervision services in the field of functional assessment and inter-sectoral cooperation, involving direct meetings with employees of Public Psychological and Pedagogical Counselling Centres (PPP) that have received a grant. The main objectives of the project are: a) the implementation of organizational changes in public psychological and pedagogical counselling centres, including specialist ones (hereinafter: PPP), resulting from the need for: - changing the PPP work model from a medical model-based approach to a biopsychosocial model-based approach, - strengthening the effectiveness of PPP cooperation with entities operating in the county, including those operating in other sectors (e.g. healthcare, social assistance), which implement actions supporting children/learners and the environments in which these persons learn and grow up on a daily basis (in particular their families, peer groups, teachers, educators and carers), - coordinating the support provided by PPP in an intra- and inter-sectoral context using the Inter-Sectoral Support Model (hereinafter: MWM) developed as part of the project implemented by the Applicant, - increasing the accessibility of PPP services, including specialist services for those in need of support, b) supplementing resources in terms of PPP equipment necessary to achieve the above objective. 2) The scope of the contract includes the provision of professional development supervision services in the amount (maximum): a) Part A: 480 hours of supervision services, 20 PPP (Kujawy-Pomerania Province) b) Part B: 648 hours of supervision services, 27 PPP (Lublin Province), c) Part C: 312 hours of supervision services, 13 PPP (Lubusz Province), d) Part D: 624 hours of supervision services, 26 PPP (Łódź Province), e) Part E: 768 hours of supervision services, 32 PPP (Małopolska Province), f) Part F: 1392 hours of supervision services, 58 PPP (Mazovia Province), g) Part G: 264 hours of supervision services, 11 PPP (Opole Province), h) Part H: 408 hours of supervision services, 17 PPP (Subcarpathian Province), i) Part I: 336 hours of supervision services, 14 PPP (Podlaskie Province), j) Part J: 480 hours of supervision services, 20 PPP (Pomerania Province), k) Part K: 888 hours of supervision services, 37 PPP (Silesia Province), l) Part L: 288 hours of supervision services, 12 PPP (Świętokrzyskie Province), m) Part Ł: 408 hours of supervision services, 17 PPP (Warmia-Masuria Province), n) Part M: 984 hours of supervision services, 41 PPP (Greater Poland Province), o) Part N: 504 hours of supervision services, 21 PPP (West Pomerania Province). 3) As part of the service provided, the Contractor will be obligated to: a) ensure procedures for collecting and storing personal data of participants in accordance with applicable law; b) adapt the time and organization (stationary or remote) of supervision sessions to the needs and possibilities of PPP employees. The method of organizing supervision sessions is dependent on the preferences of PPP employees; c) document the course of group supervision sessions; d) maintain service documentation and fulfill reporting obligations; e) prepare a regulation and obtain the Contracting Authority's acceptance of the provision of supervision services for Project participants; f) inform Supervision Participants of the need for active participation in sessions within the Project. 4) The Contracting Authority does not envisage the awarding of contracts for similar services in the sense of the provision of art. 214 para. 1 point 7 of the Pzp. 5) Contract performance period: for a period of 14 months from the date of signing the contract or until the number of supervision hours provided for in a given part of the contract is exhausted, whichever of the mentioned events occurs earlier. 6) Service delivery address: online meetings or the territory of the province (in a given part), 7) The Contracting Authority envisages the payment of remuneration in instalments for each calendar month in which supervision was conducted. Remuneration will be payable within 30 days from the date of receipt by the Contracting Authority of the invoice/VAT invoice for the performance of the service. 8) Detailed conditions for the performance of the contract and payment conditions are contained in the contract template constituting Annex No. 3 to the SWZ. 9) The Contracting Authority does not envisage additional requirements related to the performance of the contract concerning social, economic, environmental aspects, related to innovation, employment or the confidentiality of information provided to the contractor during the performance of the contract; 10) The Contracting Authority does not envisage a clause reserved in the sense of art. 94 of the Pzp; 11) Due to the type of contract, the Contracting Authority does not envisage the obligation to employ under an employment contract, as referred to in art. 95 of the Pzp. 12) The Contracting Authority does not envisage the obligation of personal performance of key tasks related to service contracts by individual contractors jointly applying for the award of the contract in the sense of art. 60 of the Pzp, nor related to the participation of entities providing resources in the sense of art. 121 of the Pzp. 13) The Contractor may entrust the performance of part of the contract to subcontractors. The Contracting Authority does not reserve the obligation of personal performance by the Contractor of key parts of the contract; the Contractor should indicate in the offer parts of the contract the performance of which it intends to entrust to subcontractors and provide (if known) the names (company) of these subcontractors. 14) A description of the criteria for evaluating offers with the indication of the weights of the criteria and the method of evaluating the offers has been detailed in Section XIII of the SWZ.
