Poland – Cardiac valve – Dostawy asortymentu jednorazowego dla Oddziału Kardiochirurgii na okres 24 miesięcy- znak sprawy Z/90/PN/26

The subject of the contract is the successive delivery of a range of single-use items for the Department of Cardio Surgery (hereinafter referred to as the goods), in the types and quantities specified in the price form constituting Annex 2 to the Contract Notice and in accordance with the planned provisions of the contract constituting Annex 4 to this Contract Notice, namely according to the following tasks:

Task 1: Delivery of biological valves;

Task 2: Delivery of surgical felt;

Task 3: Delivery of femoral arterial cannulas with an insertion set;

Task 4: Delivery of aortic valves;

Task 5: Delivery of ventricular assist devices for beating heart coronary operations;

Task 6: Delivery of bovine pericardial transcatheter heart valves;

Task 7: Delivery of biological aortic heart valves with enhanced durability for younger patients;

Task 8: Delivery of transcatheter heart valves, transfemorally (TF), in accordance with the detailed description of the subject of the contract contained in the Price Form constituting Annex 2 to the Contract Notice. The Contractor may submit an offer for one, several, or all parts. During the examination and evaluation of offers, the Purchaser will consider each task separately. The Purchaser informs that in the subject procedure conducted in the open tender procedure, it envisages the possibility of applying the procedure referred to in Article 139(1) of the Public Procurement Law. The Purchaser, by applying the aforementioned procedure, will first carry out the examination and evaluation of offers, and then carry out the qualification of the Contractor, whose offer was the highest rated in terms of the lack of grounds for exclusion and the fulfilment of the conditions of participation in the procedure.
