The subject of this tender procedure is: Purchase of a Physician Assistant integrated with the HIS system from Clininet, hereinafter referred to as the “Software” in accordance with the type and quantities specified in the “Price and Product Schedule” which is Annex 2 to the SWZ and in accordance with the “Offer Form”, which is Annex 1 to the SWZ, and the Subject of the Order Description, which is Annex 1.a and Annex 1.b – Warranty and Service Conditions for the Independent Public Health Care Unit of the University Clinical Hospital No. 2 of the Medical University of Lodz, ul. Żeromskiego 113, 90-549 Łódź, the Independent Public Health Care Unit of the University Clinical Hospital No. 1 named after Norbert Barlicki of the Medical University of Lodz, ul. Kopcińskiego 22, 90-153 Łódź and the Independent Public Health Care Unit of the Central Clinical Hospital of the Medical University of Lodz, ul. Pomorska 251, 92-213 Łódź. In the Price and Product Schedule, the quantities that will be ordered by the Independent Public Health Care Unit of the University Clinical Hospital No. 2 of the Medical University of Lodz, the Independent Public Health Care Unit of the University Clinical Hospital No. 1 named after Norbert Barlicki of the Medical University of Lodz and the Independent Public Health Care Unit of the Central Clinical Hospital of the Medical University of Lodz are indicated in the appropriate columns. Each of the Ordering Parties will sign a separate contract for the placement of the order in the quantity specified in the Price and Product Schedule for the given hospital. The implementation of the subject of the procedure is co-financed from the funds of the Medical Research Agency within the competition nr ABM/2023/2 Creation and development of Regional Centres of Digital Medicine.
