Poland – Medical equipments, pharmaceuticals and personal care products – Dostawa jednorazowych wyrobów medycznych sterylnych dla USK-2 PUM w Szczecinie.

1. The subject of the contract is the supply of single-use sterile medical devices for USK-2 PUM in Szczecin. 2. Whenever the term "products" is used in the further part of the SWZ, it should be understood that this term includes all products indicated in Annex 2 (FCJ) to the SWZ. 3. The description of the subject of the contract is contained in the unit price form (Annex 2 to the SWZ, hereinafter also referred to as "FCJ") and should be submitted together with the offer form (Annex 1 to the SWZ, hereinafter also referred to as "FO"). 4. The Contracting Authority requires the offer of medical devices – within the meaning of the Act of 7 April 2022 on medical devices (Dz.U.2024.1620 with later amendments) – introduced into circulation and use on the territory of the Republic of Poland in accordance with the applicable provisions. 5. The quantities of products indicated in the unit price form are indicative and the Contracting Authority has the right to order smaller quantities, provided that they are not less than 60% of the value of the contract/task. 6. The Contracting Authority, pursuant to Article 30(2) of the PZP Act, awards the contract in parts, each of which is the subject of a separate procedure. 7. The contract consists of 8 tasks. 8. The Contracting Authority requires that the delivered products have a minimum shelf life of 24 months from the date of delivery to the Contracting Authority's premises. 9. Submitting variant offers and those covering only some positions from the task is not allowed. 10. The contract implementation period is 36 months from the date of signing the contract.
