I. 1. The subject of the contract is the preparation of documentation for the development of protection plans for the nature reserves Cisy in Wyskitna, Jaworzyna na Chełmie, Kozie Żebro, Markowiec-Gródek, Kleszczowskie Wąwozy, Cieszynianka, Kozie Kąty, along with the preparation of protection plan projects for these reserves. The subject of the contract has been divided into 3 parts: Part I: preparation of documentation for the development of protection plans for the nature reserves Cisy in Wyskitna, Jaworzyna na Chełmie, along with the preparation of protection plan projects for these reserves. Part II: preparation of documentation for the development of protection plans for the nature reserves Kozie Żebro, Markowiec-Gródek, along with the preparation of protection plan projects for these reserves. Part III: preparation of documentation for the development of protection plans for the nature reserves Kleszczowskie Wąwozy, Cieszynianka, Kozie Kąty, along with the preparation of protection plan projects for these reserves. I.2 Description of the subject of the contract in Annex No. 9 to SWZ. II. The offer in the procedure is submitted, under pain of invalidity, in electronic form, i.e. in the form of an electronic document with a qualified electronic signature. III. 1. Documents submitted by the contractor together with the offer: 1) completed Offer Form (Annex No. 1 to SWZ). If the Contractor wishes to obtain additional points in the second tender evaluation criterion - completed list of additional expert experience (Annex No. 1A to SWZ); 2) power of attorney or other document confirming the authorization to represent the contractor – if the offer or the declarations or documents attached thereto are signed by a proxy. 3) power of attorney to represent all Contractors jointly applying for the award of the contract, or alternatively, a cooperation agreement, from which the subject matter power of attorney will result. 4) JEDZ (in accordance with the instruction from Annex No. 2 to SWZ), valid on the date of submission of offers, for the temporary confirmation that the Contractor is not excluded from the procedure on the basis of Article 108(1) and Article 109(1)(4) of the Pzp Act and meets the conditions for participation in the procedure specified in the SWZ. In the case of joint application for the contract by Contractors, JEDZ is submitted by each of the Contractors jointly applying for the contract; 5) JEDZ concerning other entities, on whose resources the Contractor relies, for the temporary confirmation, valid on the date of submission of offers, of the absence of grounds for exclusion on the basis of Article 108(1) and Article 109(1)(4) of the Pzp Act and the fulfilment of the conditions for participation in the procedure specified in the SWZ, to the extent to which the Contractor relies on their resources; 6) a declaration by the Contractor, valid on the date of submission of offers, for the purpose of confirming that the Contractor is not excluded from the procedure on the basis of Article 5k of Council Regulation (EU) 2025/2033 and Article 7(1) of the Act on special solutions in the field of counteracting the support for aggression on Ukraine and serving the protection of national security, in accordance with the template constituting Annex No. 8 to SWZ. In the case of joint application for the contract by Contractors, the declaration is submitted by each of the Contractors jointly applying for the contract; 7) a declaration by another entity, on whose resources the Contractor relies, submitted for the purpose of confirming, valid on the date of submission of offers, that the entity is not excluded from the procedure on the basis of Article 5k of Council Regulation (EU) 2025/2033 and Article 7(1) of the Act on special solutions in the field of counteracting the support for aggression on Ukraine and serving the protection of national security, in accordance with the template constituting Annex No. 8 to SWZ; 8) in the case where the Contractor relies on the capacities or situation of entities providing resources: evidence confirming that the Contractor, in implementing the contract, will have the necessary resources of other entities, which in particular constitutes the obligation of these entities to make the necessary resources available to the Contractor for the purpose of implementing the contract or another evidential means confirming that the Contractor, in implementing the contract, will have the necessary resources of these entities. 9) a declaration by Contractors jointly applying for the award of the contract, from which it follows which supplies or services will be performed by the individual Contractors, in accordance with the template constituting Annex No. 7 to SWZ (if applicable); V. The Contractor is excluded from the procedure for the award of the contract, with the reservation of Article 110(2) of the Pzp Act, on the basis of: Article 108(1) of the Pzp Act, Article 109(1)(4) of the Pzp Act and on the basis of: Article 5k of Council Regulation (EU) 833/2014 of 31 July 2014 concerning restrictive measures in view of Russia's actions destabilising the situation in Ukraine, added in Article 1(23) of Council Regulation (EU) 2022/576 of 8 April 2022 amending Regulation (EU) No 833/2014 concerning restrictive measures in view of Russia's actions destabilising the situation in Ukraine together with Council Regulation (EU) 2025/2033 of 23 October 2025 amending Regulation (EU) No 833/2014 concerning restrictive measures in view of Russia's actions destabilising the situation in Ukraine (hereinafter: Regulation 2025/2033), and Article 7(1) of the Act of 13 April 2022 on special solutions in the field of counteracting the support for aggression on Ukraine and serving the protection of national security... hereinafter SWZ
