Description

The subject of the contract is the purchase of an extracorporeal circulation device for Copernicus PL Sp. z o.o. in Gdańsk. The subject of the contract consists of 1 part. 2. The detailed scope and description of the subject of the contract, estimated quantities and the method of contract execution are contained in the annexes to the Specification of Contract Terms (hereinafter referred to as SWZ), namely: 1) Annex 1a to SWZ – Price List, 2) Annex 1b to SWZ – Description of the Subject of the Contract, 3) Annex 3 to SWZ – Draft Contract. 3. The detailed description of the subject of the contract specifying the requirements of the Purchaser and quantities is contained in Annex 1b to SWZ – Description of the Subject of the Contract. The Contractor, by completing the indicated positions in this annex, confirms compliance with the requirements specified by the Purchaser. 4. The method of executing the subject of the contract is specified in the draft contract – Annex 3 to SWZ - single delivery. 5. The offered subject of the contract must be approved for sale and use in accordance with the applicable legal provisions. 6. The subject of the contract must comply with the Act of 7 April 2022 on medical devices (i.e. Journal of Laws 2022.974 of 2022.05.09), if the Purchaser requires the offer of a range constituting a medical device. 7. In all provisions of the SWZ and its annexes, where the Purchaser refers to standards, approvals, technical specifications or reference systems or indicates trademarks, parameters or sources of origin (names of manufacturers or devices), in accordance with Article 99(5) and Article 101(4) of the Pzp Act, the Purchaser allows equivalent solutions. 8. The Purchaser does not reserve the right to tender exclusively by Contractors, as mentioned in Article 94 of the Pzp Act. 9. The Purchaser does not impose requirements, as mentioned in Article 96(2)(2) of the Pzp Act.
