Poland – Construction work – NADBUDOWA LĄDOWISKA DLA ŚMIGŁOWCÓW PRZY SZPITALNYM ODDZIALE RATUNKOWYM W SZPITALU SPECJALISTYCZNYM IM. J. K. ŁUKOWICZA W CHOJNICACH

The subject of the contract is the EXTENSION OF THE HELIPAD FOR HELICOPTERS AT THE EMERGENCY DEPARTMENT OF THE SPECIALIST HOSPITAL NAMED AFTER J. K. ŁUKOWICZ IN CHOJNICE. The detailed description of the subject of the contract is the project documentation attached to the SWZ. A mandatory local vision in accordance with the rules described in Chapter V of the SWZ. 4. The required evidential means from the Contractor include: 1) Information from the National Criminal Register in the scope of: 2) art. 108 para. 1 pt. 1 and 2 of the Pzp Act, 3) art. 108 para. 1 pt. 4 of the Pzp Act, regarding the decision to ban participation in public procurement as a criminal measure, 4) art. 109 para. 1 pt. 2 lit. a and lit. b of the Pzp Act, regarding punishment for an offence for which a penalty of arrest was imposed, drawn up no earlier than 6 months before its submission. 2) A declaration by the Contractor, within the meaning of art. 108 para. 1 pt. 5 of the p.z.p. Act, of not belonging to the same capital group, within the meaning of the Act of 12.07.2023 on the protection of competition and consumers (Journal of Laws of 2024, item 1616), with another contractor who submitted a separate offer, a partial offer or an application for admission to the proceedings, or a declaration of belonging to the same capital group together with documents or information confirming the preparation of the offer, partial offer or application for admission to the proceedings independently of another contractor belonging to the same capital group - Annex No. 7 to the SWZ, 3) Declarations by the Contractor of the currency of the information contained in the declaration, referred to in art. 125 para. 1 of the Pzp Act, in the scope of the grounds for exclusion from the proceedings indicated by the Contracting Authority, which are mentioned in: a) art. 108 para. 1 pt. 3 of the Pzp Act, b) art. 108 para. 1 pt. 4 of the Pzp Act, regarding the decision to ban participation in public procurement as a preventive measure, c) art. 108 para. 1 pt. 5 of the Pzp Act, regarding the conclusion with other contractors of an agreement aimed at disrupting competition, d) art. 108 para. 1 pt. 6 of the Pzp Act, e) art. 109 para. 1 pt. 2 lit. b of the Pzp Act, regarding punishment for an offence for which a penalty of restriction of liberty or a fine was imposed, f) art. 109 para. 1 pt. 2 lit. c of the Pzp Act. - Annex No. 5 to the SWZ. 4) Declarations by the Contractor in the scope of the grounds for exclusion from the proceedings, referred to in art. 5k of the Council Regulation (EU) 2022/576 of 8 April 2022 amending Regulation (EU) No 833/2014 concerning restrictive measures in view of Russia's actions destabilising the situation in Ukraine (OJ L 111/1 of 8.4.2022) and art. 7 para. 1 pt. 1-3 of the Act of 13 April 2022 on special solutions in the field of counteracting the support of aggression on Ukraine and serving to protect national security (Journal of Laws of 2025, item 514). - Annex No. 5 to the SWZ - Annex No. 6 to the SWZ (if applicable). 5) A copy or information from the National Court Register or from the Central Register and Information on Business Activity, within the meaning of art. 109 para. 1 pt. 4 of the p.z.p. Act, drawn up no earlier than 3 months before its submission, if separate provisions require registration in the register or record, in order to confirm that the Contractor meets the conditions for participation in the proceedings: 6) A list of construction works mentioned in Chapter VIII. CONDITIONS FOR PARTICIPATION IN THE PROCEDURE para. 4 subparagraph a) together with an indication of their type, value, date and place of performance and the entities for which these works were performed, and the attachment of evidence specifying whether these construction works were performed properly, where the evidence referred to are references or other documents drawn up by the entity for which the construction works were performed, and if the contractor is unable to obtain these documents for reasons independent of him - other appropriate documents - Annex No. 8 to the SWZ. 7) A list of services mentioned in Chapter VIII. CONDITIONS FOR PARTICIPATION IN THE PROCEDURE para. 4 subparagraph b) together with an indication of their gross value, subject, date of performance and the entities for which the project documentation was or is being performed, and the attachment of evidence specifying whether this project service was or is being performed properly, where the evidence referred to are references or other documents drawn up by the entity for which the services were performed, and in the case of recurring or continuous services are being performed and if the Contractor is unable to obtain these documents for reasons independent of him - a declaration by the contractor; in the case of periodic or continuous services still being performed references or other documents confirming their proper performance should be issued no earlier than 3 months before the deadline for submitting offers or applications for admission to the proceedings - Annex No. 10 to the SWZ. 8) A list of persons, mentioned in Chapter VIII. CONDITIONS FOR PARTICIPATION IN THE PROCEDURE para. 4 subparagraph c) together with information about their professional qualifications, authorisations, experience and education necessary to perform the public contract, as well as the scope of the activities performed by them and information on the basis for disposing of these persons – Annex No. 9 to the SWZ. 5. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, it submits documents described in detail in Chapter X para. 5 of the SWZ.
