Poland – Medical consumables – Dostawa mikronarzędzi oraz wyrobów medycznych do zabiegów okulistycznych
Description
The subject of the contract is the successive delivery of microsurgical instruments and medical products for ophthalmic procedures. The contract is divided into 38 lots (parts). Contracts for individual lots will be concluded after the completion of this procedure for a period of 12 months. The detailed description of the subject of the contract and the requirements of the Purchaser regarding the implementation of the contract are specified in: a) The delivery specification is contained in Annex No. 1 to the SWZ (Tender Form), and the detailed characteristics are Annex No. 1.1 (Price Form) for individual lots, b) The draft contract terms – Annex No. 2 to the SWZ. All the requirements specified in the above documents are minimum requirements and their fulfillment is mandatory. Failure to meet the above minimum requirements will result in the rejection of the offer as non-compliant with the terms of the contract. Each contractor may submit an offer for one or any number of selected lots, but within a given lot, the offer must be complete (i.e., it must include all items), otherwise it will be rejected. The Purchaser does not require the submission of offers for lots for which the Contractor does not submit an offer. The Purchaser does not envisage the allocation of individual positions from the lots established in this SWZ during the tender procedure. The Purchaser does not allow the submission of offers for randomly selected positions in a given part. In individual parts, the full assortment must be priced in the quantities specified by the Purchaser. Annex No. 1.1 ("Price Form") must be completed in full, without making any changes to its content – it is an integral part of the offer – the Contractor's declaration regarding its content, therefore the absence of this annex, containing content consistent with the template specified in the SWZ, will result in the rejection of the offer (after the Purchaser has performed all the actions provided for in the Pzp Act in the scope of the assessment of offers).
