The subject of the Contract are insurance services against the consequences of accidents for the benefit of Units, that is, for the benefit of organizational units subordinate to or supervised by the Minister of Justice, in the following scope: Part I - group insurance against the consequences of accidents for sentenced persons performing unpaid, supervised work for social purposes and socially useful work in accordance with the provisions of the regulation issued under art. 56a § 2 of the Act of 6 June 1997 - the Penal Code Execution Code (consolidated text Journal of Laws of 2025, item 911, as amended), including in accordance with the provisions of the regulation of the Minister of Justice of 27 May 2010 on the procedure for concluding by the entitled entities of the insurance contract for the consequences of accidents for sentenced persons performing unpaid, supervised work for social purposes and socially useful work (Journal of Laws of 2010, No. 98, item 632, as amended), during the period of validity of this regulation; Part II - group insurance against the consequences of accidents for minors performing social work in accordance with the provisions of the regulation issued under art. 152 para. 5 of the Act of 9 June 2022 on the support and resocialization of minors (consolidated text Journal of Laws of 2026, item 163, as amended), including in accordance with the provisions of the regulation of the Minister of Justice of 31 August 2022 on the procedure for concluding by the entitled entities of the insurance contract for the consequences of accidents for minors performing social work (Journal of Laws of 2022, item 1888, as amended), during the period of validity of this regulation; Part III - group insurance against the consequences of accidents for persons placed or placed in Correctional Facilities, Shelters for Minors, Regional Educational Centers and Guardianship Centers, as well as insurance for the consequences of accidents and civil liability insurance for persons performing work or activities in Guardianship Centers. 2. The subject of the contract has been divided into 3 parts. The Purchaser allows the possibility of submitting partial offers for one, two or all parts of the contract. A partial offer must include the full scope described for the given part of the contract. 3. A detailed description of the subject of the contract for each part has been specified in: a) the templates of the Agreements constituting respectively Annexes No. 5a-5c to the SWZ; b) Annexes No. 1 to the templates of the Agreements "Description of the Subject of the Contract" constituting respectively Annexes No. 2a-2c to the SWZ. 4. The Purchaser, acting under art. 95 para. 1 of the Pzp Act, requires the employment by the Contractor or Subcontractor on the basis of an employment relationship of all persons who will be responsible for the service of the Contract and will perform activities involving: maintaining current contact with the Insurer, the Insured or the broker (conducting correspondence within the scope of the Contract and telephone contacts regarding the Contract), issuing insurance documents, settling the Contract, confirming the scope of the Insurer's liability, confirming insurance cover, informing about the status of insurance premium settlements, providing information or explanations related to the implementation of the Contract, on the principles and to the extent specified in the template of the Agreement constituting respectively Annexes No. 5a-5c to the SWZ. 5. The meaning of the terms used in this SWZ is identical to the definitions adopted in the templates of the Agreements constituting respectively Annexes No. 5a-5c to the SWZ and the Descriptions of the subject of the contract constituting respectively Annexes No. 2a-2c to the SWZ. 6. The list of Units for which the procedure is conducted is included in Annex No. 8 to the SWZ. 7. Detailed data on: a) claims, have been included in Annex No. 10 to the SWZ; b) the number of Insured have been indicated in Annexes No. 9a-9c to the SWZ in the form "Cost Calculation".
