Poland – Cleaning and sanitation services in urban or rural areas, and related services – „Utrzymanie czystości w mieście w latach 2027-2028 – Zapewnienie czystości w mieście poprzez świadczenie usług zewnętrznych”

The subject of the contract is the annual provision of current maintenance services, winter maintenance, and related works. The contract has been divided into two parts: 1) Part 1 - Annual Cleanliness Maintenance - the city of Nowy Tomyśl and indicated municipal and village areas a) The subject of the contract is the annual provision of current maintenance services, winter maintenance, and related works within the boundaries of Region I. The scope includes the city of Nowy Tomyśl, excluding areas assigned exclusively to Region II, and indicated objects and routes in rural areas. b) A detailed description of the subject of the contract is contained in the documentation constituting Annex 1a to SWZ. 2) Part 2 - Maintenance of lawns and green areas - Północ district, Batorego district and adjacent areas in Nowy Tomyśl a) The subject of the contract is the maintenance of lawns and green areas and related works within the boundaries of Region II. The scope includes the areas of Północ district, Batorego district, and adjacent green areas listed in the annexes. b) A detailed description of the subject of the contract is contained in the documentation constituting Annex 1b to SWZ. 2. In accordance with Article 126(1) of the PZP, the Contracting Authority will invite the contractor whose offer was highest-scored to submit, within a specified deadline, no shorter than 10 days from the date of invitation, the personal means of proof, if required to be submitted in the contract notice or contract documents, current as of the date of submission of the personal means of proof. 3. The Contracting Authority will use Article 139(1) of the PZP, i.e.: The Contracting Authority will first conduct an examination and evaluation of offers, and then conduct a subjective qualification of the contractor whose offer was highest-scored, in terms of the absence of grounds for exclusion and the fulfillment of the conditions for participation in the procedure. 4. The contractor may submit only one offer for a given part of the contract. The Contracting Authority allows the submission of partial offers. 5. A set of documents that make up the offer: 1) Completed offer form (according to the template – Annex 2 to SWZ), 2) JEDZ declaration; 3) Contractor's declaration regarding the grounds for exclusion from Article 5k of Regulation 833/2014 4) copy or information from the KRS or CEiDG, if separate regulations require registration in the register or record, in order to confirm that the person acting on behalf of the Contractor is authorized to represent him; 5) confirmation of the deposit. Additionally, the offer should be accompanied by – if applicable: 6) power of attorney authorizing the submission of the offer, if the offer is signed by a proxy; 7) power of attorney for a proxy to represent in the procedure of contractors jointly applying for the grant of the contract – applies to offers submitted by contractors jointly applying for the grant of the contract, 8) commitment of a third party; 9) JEDZ declaration for each of the entities providing the Contractor with resources, if the contractor relies on the resources of other entities; 10) declaration of the entity providing resources regarding the grounds for exclusion from Article 5k of Regulation 833/2014 (if applicable); 11) JEDZ declaration for each of the contractors jointly applying for the grant of the contract, in the case of contractors jointly applying for the grant of the contract; 12) declaration regarding the grounds for exclusion from Article 5k of Regulation 833/2014 for each of the contractors jointly applying for the grant of the contract, in the case of contractors jointly applying for the grant of the contract; 13) declaration under Article 117(4) of the PZP, mentioned in Section X(6) of SWZ (if applicable).
